Many people often wonder
what Primary Care is?

Primary care is that care provided by physicians specifically trained for and skilled in comprehensive first contact and continuing care for persons with any undiagnosed sign, symptom, or health concern (the "undifferentiated" patient) not limited by problem origin (biological, behavioral, or social), organ system, or diagnosis.

Primary care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses in a variety of health care settings (e.g., office, inpatient, critical care, long-term care, home care, day care, etc.). Primary care is performed and managed by a personal physician often collaborating with other health professionals, and utilizing consultation or referral as appropriate. Primary care provides patient advocacy in the health care system to accomplish cost-effective care by coordination of health care services. Primary care promotes effective communication with patients and encourages the role of the patient as a partner in health care.

At your MyCareClinic, our primary care physicians consider the whole person in preventative and diagnostic healthcare, rather than individual systems or organs. Our integrated model is built upon a team approach that brings together physicians, certified physician assistants, nurse practitioners, medical assistants, nurses and other clinical staff to deliver coordinated care that includes:

•Preventative healthcare

•Management of existing medical conditions

•Healthcare plans for chronic conditions

•Coordination between PCPs and specialists

•Access to and education about self-care between appointments

Our primary care physicians coordinate our patients' care and manage puzzling medical conditions associated with that care, bringing in specialists as needed. Internists combine the role of a diagnostician and a consultant in disease prevention and health promotion, providing a continuum of care for patients with physicians across various specialties when multisystem diseases occur.

MyCareClinic primary care physicians practice in the following departments:

•Internal Medicine  - Our internists see teenage patients and older, and take pride in seeing them for life.

•Family Medicine - The MyCareClinic Family Medicine Department is available to patients of all ages - from infancy through adulthood. Family members from one household may opt to see the same Family Medicine physician.

•Pediatric and Adolescent Medicine - Our pediatricians see infants, children and adolescents and young adults - and some of our providers are seeing their third generation of babies.

Primary care physicians are trained to handle all aspects of healthcare for adults, including:

•Preventative healthcare screening, including colonoscopies, mammograms, pap smears, cholesterol screening, blood pressure screening)

•Caring for patients with undiagnosed issues (chest or abdominal pain, shortness of breath)

•Coordinated care between specialties for patients with chronic medical conditions, including diabetes, heart disease and high blood pressure

If our patients require care from physicians in other specialties, or require lab testing or radiology imaging, our primary care physicians coordinate the care and consult on issues related to diagnosis, treatment and patient care.
